About the Role

As a Care Coordinator for Access Community Health, you will play a vital role in coordinating home-based care and support services that enable our clients to live safely and independently within their homes and communities. You will develop and maintain rapport with our team, clients, and their families, delivering high-quality customer-focused service within a structured framework.

Main Responsibilities:

Requirements:

  • Eligibility to work in New Zealand
