Regional Care coordinator

We have 2 exciting opportunities available at Access Community Health for a Regional Care coordinator position. We're looking for a full time (Monday - Friday) care coordinator based in our Mt Eden office, and full time based in Northland in our Whangarei office (Monday to Friday) We are looking for people with a desire to make a valuable and rewarding contribution to our communities by joining our Northern Access Community Health team.
- Requirements:

- Able to question broadly to understand individual and situational dynamics and to 'Read between the lines"
- Naturally empathetic and caring but able to maintain professional focus and observe boundaries
- Excellent general PC skills including the full Microsoft Package
- Excellent organisational skills, particularly time management and prioritising
- An eye for detail and an ability to maintain a high level of accuracy
- Excellent interpersonal skills that enable the building of effective relationships across diverse groups, including negotiation and conflict resolution skills
- Works Well under pressure and multi-tasks across functions and activities.
- Responsibilities:

- Facilitate co-operative and cohesive working relationships between care coordination and support worker teams.
- Setting up new client referrals within a specific region.
- Meet personal/team qualitative and quantitative targets
- Facilitate co-operative and cohesive working relationships between Care Coordination and Support Worker teams
- Deliver high quality, customer focused service within a structured framework and in a highly flexible service response environment
- Develop and maintain strong working relationships with support workers and service users, thus enhancing their roles and sense of belonging to Access
- To deliver services by following our Whakatauaki, "Care for our community, Work together, Look after each other and Do the right thing".
