Enliven maximises the independence of people, working to enable people to live in their own homes, stay healthy and engage socially with others.
This role will work across our Enliven Northern region, being the wider Auckland area.
This interesting role will see you utilise your skills as you support the delivery of our client-driven and flexible community based support services to the highest standards. This involves:

- Initial assessment and regular reassessment of clients in their own home
- Working collaboratively with clients, family and other agencies
- Development of individual support plans, in accordance with the clients’ specific goal-based needs
- Leading, developing and managing a team of Support Workers to deliver the support and assistance to meet clients’ goals and needs

**You are a good fit for our team if you**:

- Have a current Practising Certificate
- A proven track record in a health background
- Ability to build relationships and network
- Ability to work independently but also part of a team
- Capability to prioritise and manage your time effectively, with well developed organisational skills
- An understanding of the restorative approach to service delivery in the home care based environment
