The Role
As a Case Manager, you will provide needs-based services to individuals with complex injuries, ensuring the best possible outcomes and independence. You will determine injury or wellness-related needs, deliver initial action plans, and monitor costs associated with rehabilitation plans.

Your Skills and Experience
