**Are you an experienced healthcare professional with a passion for ensuring patients receive coordinated care that is tailored to their individual needs?**

We have an exciting opportunity now available for motivated, clinically experienced, registered health professionals to join our Clinical Improvement and Development Team as Patient Care Plan Coordinators. The Patient Care Co-ordinator works with our patients with complex needs and frequent calling to ensure they are connected within health, while easing pressure on Hato Hone St John's services and Emergency Departments through these connections. You will often be working with our most vulnerable patients.

This is a permanent position, working 40 hours a week Monday to Friday 8:30 am to 5:00 pm

In this role, you will work with patients, primary health care, ambulance personnel, community and other health professionals. You will be responsible for creating individualised response plans to meet the patient's needs ensuring we continue to offer a safe and equitable service.

**Key Responsibilities**:

- Identify through established reporting systems patients who would benefit from an individualised response
- Work collaboratively with patients and their primary healthcare providers (physicians, nurses, and other healthcare professionals) to develop individualised care plans based on medical history, current conditions and unique needs, whilst ensuring each care plan is comprehensive, safe and fit for purpose
- Communicate with the patient, their health care provider/s and other health care professionals to ascertain the need for an individualised response plan
- Regular reviewing of plans within the PCP database to ensure PCPs are updated or removed as required
- Engaging with internal and external stakeholders to ensure a completed shared vision is achieved
- Demonstrate behaviours that align with St John's core values - **_Do the right thing, Side by side, Straight up, Make it better, Open minds_**
- Be a registered health professional with relevant experience and current annual practising certificate
- Experience in either social work, nursing and or mental health
- Be able to work both autonomously and as part of a team
- Patient-centred professional/clinical practice
- Well-developed IT skills, experience in the use of patient management databases is desirable
- Broad knowledge and appreciation of the challenges facing health and social care
- Excellent written and verbal communication skills
