**Te whiwhinga mahi | The Opportunity**

The NZ Familial Gastrointestinal Cancer Service (NZFGCS) is a Ministry of Health funded national service with offices in Auckland, Wellington and Christchurch. Our multidisciplinary clinical team of Family History Assessors, Gastroenterologists and Colorectal Surgeons identify, assess and manage people with familial gastrointestinal (GI) cancer syndromes. The team also provides education, information and support to families, including medical management advice and a colonoscopy surveillance referral programme.

**We are seeking expressions of interest for a full time Patient Intake Coordinator. **This role requires a health science degree; significant experience working with patients and a genuine interest in supporting families with inherited cancer syndromes. You will need to be comfortable working with clinical data.

Appointment at a level appropriate to qualifications and experience is offered and training will be provided.

**In this role, you will**:Support the patient and their family through an assessment pathway to determine the level of familial cancer risk.
- Explain the role of the Service to the patient;
- Gather family cancer history information from the patient and support the patient to fill out a questionnaire to collect pertinent information
- Obtain consent from the patient and then request reports from hospital and laboratory investigations
- Arrange investigative tests in liaison with Family History Assessors and Medical Advisors.

This role directly supports the patient journey by facilitating assessment for the Triage Clinician; providing information to support the subsequent identification of specific hereditary cancer syndromes by the Family History Assessor assigned to the case

You will need to demonstrate an awareness of the ethical issues related to the medical management of genetic disorders and the psycho-social impact on people of a diagnosis of cancer or who are at risk of developing cancer
