**A exciting role to support the transition of tangata whaiora from the Forensic Service to the Community.**:

- ** Fixed Term full-time position for 1 year**

**Mō te tūnga | About the role**

Forensic mental health services specialise in the assessment, treatment and risk management of people with a mental disorder who are currently undergoing, or have previously undergone, legal or court proceedings.

This position is a clinical position allocated specifically to identify and then work with tangata whaiora and staff to transition inpatient tangata whaiora through to community placements by targeting barriers. You will be working closely with tangata whaiora and their whanau, NGO partners and Forensic Inpatient and Community MDTs.

**Ko wai koe | About You**
- Establishing self as a key member of the MDT to support with identifying the specific needs of each of the tāngata whaiora who currently have no discharge pathway.
- Working with planning and funding and community providers to identify where needs can be met including looking at alternate care pathways.
- Working with tāngata whaiora and their whānau to support them as required through the discharge process.
- Development of comprehensive transition plans which include the consumer, MDT members, whānau and support agencies
- Assisting with the transition process
- supporting the tāngata whaiora, their wahnau, the MDT and the community provider

**Ngā pūkenga me ngā wheako | Skills and experience**
- Ability to engage and work with clinical MDT teams to identify and address individual tāngata whaiora needs and to identity appropriate discharge pathways including any barriers to discharge.
- Ability to develop and maintain therapeutic relationships with tāngata whaiora and their whānau and work in partnership with them to ensure successful and appropriate discharge pathways.
- Ability to work collaboratively with various stakeholders including clinical inpatient / outpatient teams and ROSC.
- Comprehensive knowledge of community providers, and the ability to develop and maintain strong working relationships with community agencies and providers
- Ability to problem solve and think creatively
- Will be resilient and adaptive - shows composure, resolve and a sense of perspective when the going gets tough. Helps others maintain optimism and focus
- Honest and Courageous - delivers clear messages and makes decisions in a timely manner; to advance the longer term best interests and drive improvement in processes, equity and outcomes for tāngata whaiora and whānau.
- Ability to provide training and support to community providers on the specific needs of the consumer.
- Culturally responsive - works proactively with Māori to uphold the principles of Te Tiriti ō Waitangi and implements the DHB’s vision of ensuring equitable outcomes for Māori

Essential Experience:

- NZ registration with the relevant professional body (allied health or nursing)
- Minimum five years’ experience as a clinician in mental health services
- Experience in people leadership and or management
