Mō te tūnga - About the Role
The Canterbury Hauora Co-ordination Hub was originally
established to support people with the COVID 19 Omicron variant to safely
isolate. The Hub continues to do this work but also provides a range of other
services to support both primary and secondary services. These include work to
avoid unnecessary admissions, support earlier discharge and assist primary and
community agencies to keep people with health and / or social complexities,
well in their own homes. The Hub - s - 5 C - s - model of care is working
with Complexity, using the skills of Communication and Co-ordination
with the goal of Connecting people
to supports and resources - always with a lens to identify and find ways to
meet Cultural support needs. Much of our work is with people who are not currently
enrolled in General Practice. We are also further developing a - Long COVID
- project to co-ordinate the work and support those impacted by delayed recovery
from this illness. We are especially seeking those who have experience in fatigue
management, respiratory care and / or community rehabilitation for this work.
We are co-located with a small team from the Ministry
of Social Development and work collaboratively (with client consent) to bridge
the gaps where both agencies are involved. The bulk of our work is virtual /
telehealth but may also include face to face work in people - s homes or at
appropriate community locations. Alongside the clinical elements, a significant
proportion of the role will be non-clinical including role scoping, further
service development and ongoing service improvements.

Expressions
of interest are sought for up to a total of 3.5FTE registered allied health
professionals from Physiotherapy, Occupational Therapy, Dietetics or Social
Work to join the existing allied health team at the Canterbury Hauora
Coordination Hub. These roles will include some FTE to cover 7 days per week
and would be either fixed term secondments or appointments until 30 June 2023.
Consideration will be given to those able to work between FTE.
A primary focus of our work is addressing health
inequities and you
will work closely with health colleagues and our Equity partners (Tangata
Whenua, Pasifika, CALD, Rainbow and Disability) and other agency teams to
ensure members of our community and their whānau have their health and social
service needs met.
The successful applicant(s) will have a demonstrated commitment to
addressing equity issues, and proven ability to work with diversity. You will
be a culturally competent team player, relationship and solutions focused,
adaptable and able to engage quickly and effectively with a wide range of
people needing our services and an effective communicator with all parts of the
health and welfare system.
A commitment to strong interdisciplinary ways of working and
effectively working with allied health assistants are also essential.
