**Service Coordinator.**

The
**Service Coordinator** is a pivotal role for Care on Call (NZ) Limited and our clients. As an organisation, we support people throughout New Zealand who are recovering form illness, injury, living with disability and/or needing support or care in their homes. We offer support through our awesome homecare support workers and nurses as well as supporting people managing their own individualised funding.

This particular role is based in our Napier office, Monday to Friday and generally covers the coordination of services for our clients in Hawkes Bay region but also back up support for our nationwide service when needed.

Day to day you will be:

- reviewing referrals about clients from the hospitals/ ACC providers
- taking calls from people wanting some help at home
- loading data and details into our swish database system
- organising rosters, matching support staff with clients based on their needs/skills
- updating clients and staff about rosters and services
- responding to changes
- following up and checking in on how cares are going
- managing budget or expenditure requests
- providing support for people to achieve their identified goals and dreams

To be considered you will need to:

- come with awesome spoken and written English
- have attention to detail
- be a decent person, who cares about the wellbeing of others
- may have healthcare / disability sector or service coordination experience (this would help hugely too)
