Job Summary

The Patient Services Rep will play a vital role in supporting the administrative and scheduling needs of our Innovative Delivery Programs. This includes Population Health at Home, Remote Patient Monitoring, Complex and Transitional Care, and other population health programs/initiatives as needed.

  • Respond to phone calls, voice mail messages, and monitor front desk pools, documenting events in patients' digital health records.
  • Fax, scan PHI into digital health records, and sort mail.
  • Schedule and confirm patient appointments, perform check-in and check-out procedures.
  • Prepare patient charts and information prior to scheduled appointments.
  • Activate patients' MyChart accounts if not active.
  • Support patient screening for digital remote patient monitoring (RPM) program eligibility and facilitate enrollment.
  • Order and ship digital remote monitoring devices, confirming patient receipt.
  • Anticipate problems or issues and solve within scope of responsibility.
  • Review digital RPM patient agreements and ensure signatures.
  • Collaborate with patients, caregivers, providers, and other healthcare team members.
  • Maintain patient confidentiality and privacy.
  • Participate in regular in-services to enhance position and department knowledge.
