About the Role

As a Disability Support Coordinator, you will champion the needs of our clients and drive positive outcomes. You will provide professional support, conduct needs assessments, reassessments, and arrange support packages for clients.

Key Responsibilities
  • Conduct in-depth needs assessments to identify and prioritize client support needs
  • Coordinate tailored support packages that meet individual goals and needs
  • Collaborate with stakeholders, team members, and providers to ensure client needs are understood and achievable
Requirements

To be successful in this role, you will need:

  • Strong verbal and written communication skills with the ability to relate to people from diverse cultural backgrounds
  • Proven experience in coordinating natural and formal supports and services for people with a range of physical, intellectual, and sensory needs
  • Commercially savvy and capable of navigating operational IT systems
