Job Description

We're seeking a skilled Clinical Care Specialist to join our team. As a key member of our healthcare team, you'll work collaboratively with GPs and other clinicians to provide assessment, diagnosis, planning, implementation, and evaluation of treatment for our patients.

Key Responsibilities
  • Collaborative assessment and diagnosis of patient needs
  • Development of personalized treatment plans
  • Supporting patients to adopt healthy lifestyles and self-care practices

Requirements
  • NZ Nursing Registration and post-graduate education in prescribing
  • Ability to support the cultural beliefs and aspirations of tūroro and whānau
