**Te whiwhinga mahi | The Opportunity**

We are seeking an enthusiastic, passionate and skilled registered health professional to take the opportunity to work as an Associate Charge Nurse Manager or Associate Team Lead in the Disability Support Link team.
- In this senior leadership role you will be working alongside the CNM to provide leadership and support to the team; prioritising and coordinating patient assessments, leading the interdisciplinary team, managing client queries/support, and adhering to the many different contracts and service development.

**Key responsibilities of the role**:

- Support the Charge Nurse Manager.
- Lead others to provide an effective practice environment.
- Assist with induction, coaching and supervision.
- Assist with direct care as required (for high and complex or urgent cases).

**Mōu | About you**
- Skills and experience you’ll need to be considered for the role:

- Commitment to self-development and the development of others.
- Full New Zealand drivers licence.
- Holds a current NZ annual practicing certificate.
- Strong teamwork and a collaborative leadership approach.
- Proven previous leadership role.
- Robust experience of 5 years or more, working within the community and with older people.
- Able to role model.
- Experience in service development.
- Post graduate qualification.
- InterRAI Home Care competency is an advantage.

**Mō tātou Ratonga | About our Service**
- Disability Support Link covers a wide geographical area with nine satellite offices spread across the Waikato region.
- We work with Home and Community Service Providers, Age Related Residential Care facilities, Primary Care and inpatient teams to support approximately 10000 clients across our Motu.
- We complete interRAI assessments and coordinate long term care in the community and aged residential care.
- We are developing a care management model where assessor coordinators will be proactively supporting a caseload and follow up ED presentations and prevent hospital admissions for clients.
