Are you passionate about the disability community? Do you want to utilise your skills and knowledge of the health or disability sector in a rewarding role within a collaborative and supportive team?
As a Pathways Navigator, you will primarily be assessing referrals to ensure those who need our support receive access to the relevant services. Your detail-oriented holistic approach will ensure eligibility is assessed efficiently and accurately, to facilitate people being able to access the supports they need across a wide range of services and providers.
We are Kaikaranga. Since 2002, we've been helping people with disabilities access an array of services to live a good life.
**About the role **
- Review new referrals to determine whether the person is eligible for assessment under disability support services
- Provide information, education, and advice to potential clients and referrers about eligibility, supports and potential services
- Action queries promptly and efficiently ensuring effective resolution or escalation as required
- Proactively seek information to establish eligibility and assess urgency and risk
- Accurately document outcomes and complete escalation responses as required
- Support people who are not eligible for our service to access other services that may support them
**About you **
- Passionate about what you do and committed to Kaikaranga values
- A sound working knowledge of the health and disability sector, ideally with a clinical background
- The ability to maintain efficiency and a positive attitude when juggling a busy day
- A great team player who can work efficiently and autonomously
- Exceptional communication, and customer service skills, with the ability to relate to a diverse range of people
