**About the role**:
An exciting opportunity has arisen to join the Health Care Home team for Collaborative Aotearoa hosted by Tu Ora Compass Health as a Change Lead: Health Care Home and Digital Health. Health Care Home (HCH) is a model of care based around general practice and designed to improve the quality and sustainability of services as well as the experience of both patients and staff and includes digital health technologies to support access to primary care.

The Change Lead's primary responsibility will be to provide change leadership and facilitation to support the HCH model of care and the roll out of digital health across general practice.

**Skills and Abilities needed for this role**:

- Experience building excellent relationships and managing stakeholders
- Understanding of change and lean philosophies
- Ability to facilitate change processes in an operational environment
- Demonstrates a strategic approach when mitigating risks
- Strong analytical and critical thinking skills
- Accomplished in the area of planning, goal setting and co-ordination of tasks
- Wide range of experience and knowledge in Microsoft Suite and other data management platforms

**About you**:

- You will be a motivated self-starter who thrives on creating innovative ways to influence change
- Able to work well in a team as well as autonomously
- Uses an equity lens over all work to ensure it contributes to improved health outcomes for Maori and other priority populations.
- Project and/or Change Management experience
- Knowledge and practical experience in process improvement and Lean methodology (preferred)
- Primary Care and/or health background (preferred)
- Must be a NZ citizen, hold a permanent resident visa or have the right to work in New Zealand as we are not an accredited employer
