Te Whatu Ora - Health New Zealand, Hauora a Toi Bay of Plenty is spread across a vast geographical area and centred across two hospitals based in Tauranga and Whakatāne. We serve a population of approximately 259,000 for the major population centres of Tauranga, Katikati, Te Puke, Whakatāne, Kawerau and Ōpōtiki. Te Whatu Ora Hauora a Toi Bay of Plenty is one of the Bay of Plenty's largest employers with over 4000 staff.

**He Kōrero mō te tūranga - About the Role**

The Regional Team Leader (Health of Older People) leads a team of 31 staff, with 7 direct reports who undertake Needs Assessment and Service Coordination (NASC) services in the Bay of Plenty.

The Regional Team Leader is accountable for the services the team delivers and the results they achieve, as well as the relevant financial budgets. The team delivers (NASC) services to older people and people of all ages who have a chronic health condition, living in the community.

The Regional Team Leader is also an active contributor to the overall Support Net management group and has input into strategic and operational decisions.

This is a full-time opportunity working 40 hours per week based in the Bay of Plenty Community.

**He korero mōu - About You**

We are looking for someone with a professional qualification that is relevant to delivering NASC services. Ideally you will have worked in a NASC agency (or similar service) and understand the complexities of delivering NASC services to a diverse range of clients.

You will be creative, innovative and an expert problem solver with an ability to encourage and motivate a large team. You will have knowledge and understanding of the community provider sector and be able to develop and build on strong community relationships.
