Value-Based Care Specialist

Job Summary:
The Population Health Manager will develop and oversee the strategic direction of payer value-based programs. This individual will maintain relationships with internal and external stakeholders, work to complete projects within agreed-upon timeframes, and identify and escalate any risks, mitigation plans, and constraints.
Key Responsibilities:
* Develop and implement strategies to support value-based care initiatives
* Collaborate with business and clinical partners to create tactics, approach, and timelines for projects
* Support change management processes to facilitate smooth transitions in the organization
* Contribute ideas for enhancing value-based programs and communicate updates to stakeholders
* Assist in data collection and analysis related to value-based metrics
* Patient chart review, validation of quality measures, and partnership with practice operations to support quality improvement
Requirements:
* 3-5 years' experience in healthcare program management
* BS/BA preferably in business, healthcare, operations, or nursing
* Master's degree in healthcare, finance, or organizational development preferred
* Lean Six Sigma Yellow, Green, or Black Belt - Preferred
* Change agent - Preferred
* Project management - Preferred
