Care Coordinator

We are seeking a highly motivated and empathetic Support Worker to join our Supported Living Team. In this rewarding role, you will have the opportunity to make a positive impact on the lives of individuals with intellectual disabilities living in their own homes.
The successful candidate will be responsible for providing individualized support to enable our clients to achieve their goals and aspirations. This may involve assisting with daily living skills, such as cooking, cleaning, and budgeting, as well as facilitating community participation and social connections.
To be successful in this role, you will require excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues. You will also need to be adaptable, flexible, and able to work independently with minimal supervision.
