The RoleWe are seeking a registered health care professional (Registered Nurse, Occupational Therapist or Social Worker) to join our new community health team. The primary purpose of the role is to facilitate transitions throughout the service system, improve information flow, and monitor systems and processes that underpin good quality care and support.
- Key Responsibilities
- Work with patients, their whanau and providers to produce plans together, to best meet their needs and preferences
- Coordinate plans to direct, enable and evaluate interventions from various providers to meet goals identified
- Develop strong interagency relationships across Iwi, Maori NGO providers, Government organizations, Non-Governmental Organizations and the Mental Health and Addictions Sector
About YouCandidates must have competency with te ao Maori, tikanga, and te reo Maori, or a commitment to starting their journey and taking ownership of their learning and growth. They must also be whanau centred, collaborative, and partnership-focused, with experience working in mental health, community health or primary care settings. A highly developed ability to build productive working relationships with colleagues is essential.
