Job Overview

We are seeking an experienced mental health professional to join our community team as a Senior Mental Health Care Coordinator. As a key member of the team, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Main Responsibilities

  • To provide assessments and develop treatment plans for service users on caseload.
  • To act as named practitioner and coordinate care between services.
  • To hold a caseload of people with severe and enduring mental health problems.
  • To participate in team meetings, MDT meetings, and training opportunities.
  • To provide advice and support for service users and carers.
  • To participate in the delivery of group interventions.
  • To supervise junior colleagues in the team.
  • To liaise with colleagues and coordinate care for service users on caseload.

Requirements

  • Experience of working in an acute environment.
  • Experience of mentorship, supervision, and coordination of staff.
  • Experience of working as a Nurse in Charge/Shift Coordinator.
  • Experience of Care Planning process.
  • Working in a multi-agency framework.
  • Demonstrate a contribution to change management processes.
  • Experience in positive leadership of a team.
  • Demonstrate experience of problem-solving approaches.

Person Specification

  • Up to date Professional Registration (to be maintained), on the appropriate part of the NMC register and hold a current NMC registration.
  • Post-registration training in relevant field.
  • Leadership/Management training.
