Care Coordinator for Mental Health

About the Opportunity

We are seeking a skilled Care Coordinator to join our team at the North Adult Community Psychiatric Service at Hillmorton in the Specialty Mental Health Service. In this role, you will work closely with consumers and their families to coordinate care and support services.

Your Responsibilities

You will assess consumers' needs and develop individualised care plans that promote recovery and well-being. You will also work collaboratively with healthcare providers and community agencies to ensure that consumers receive comprehensive care and support.

Our Team

The North Adult Community Psychiatric Service has a multidisciplinary team that includes Psychiatric Nurses, Consultant Psychiatrist, Occupational Therapists, Clinical Social Workers, Psychologists, Registrar, and administration staff. We work together to provide comprehensive care and support to consumers.

To succeed in this role, you will need:

  • A current clean driver's license - preferably able to drive both manual and automatic
  • Certificate in Mental Health studies, or other training in Mental Health
  • Experience in Community Mental Health delivery
