About the Opportunity
We are looking for a skilled Community Care Worker to join our team at the North Adult Community Psychiatric Service at Hillmorton in the Specialty Mental Health Service. As a Community Care Worker, you will provide support and assistance to individuals in their own homes or in the community.
Your Role
In this role, you will work with consumers who are disabled by mental illness to enhance their independence and quality of life. You will collaborate with Case Managers to develop and implement individualised care plans that promote recovery and well-being.
Our Team
The North Adult Community Psychiatric Service has a multidisciplinary team that includes Psychiatric Nurses, Consultant Psychiatrist, Occupational Therapists, Clinical Social Workers, Psychologists, Registrar, and administration staff. We work together to provide comprehensive care and support to consumers.
To succeed in this role, you will need:
- A current clean driver's license - preferably able to drive both manual and automatic
- Certificate in Mental Health studies, or other training in Mental Health
- Experience in Community Mental Health delivery
