* Assist the team leader in prioritising clinical workloads and coordinating patient care plans
* Develop and maintain strong relationships with healthcare providers and community organisations
* Identify opportunities for process improvements and implement changes as needed
* Participate in professional development activities to enhance knowledge and skills

About the Team/Service
Our team works primarily with older adults who have been admitted to hospital with a medical condition, following a stroke, or as a result of an injury. We aim to facilitate quicker discharges from hospital and provide nursing and allied health input/rehabilitation services to patients in their own homes.
