Care Assessment and Coordination Lead

We are seeking a highly skilled Care Assessment and Coordination Lead to join our team at NHS Cheshire and Merseyside Integrated Care Board. As a key member of our commissioning team, you will be responsible for assessing and coordinating care for adults and young people with complex health needs.

Key Responsibilities

  • To assess and coordinate care for adults and young people with complex health needs, ensuring timely and robust ongoing reviews are completed.
  • To plan, commission, and procure bespoke care packages for eligible individuals, embedding personalisation into care and support plans.
  • To provide ongoing case management, working collaboratively with patients, carers, and professionals across health and social services to ensure seamless care transitions.

Requirements

  • A degree or equivalent level of experience in health or social care, with current NMC registration.
  • Experience in commissioning adult complex care packages and awareness of the integrated health and social care commissioning agenda.
  • Strong leadership skills, with the ability to work collaboratively with stakeholders and implement national policy.
