Care Coordinator Role
The Care Coordinator plays a vital role in ensuring the highest quality of care and support for our residents. As a key member of our team, you will be responsible for coordinating Caregiver activities across each day, escalating resident care needs to Nurses on shift and Care Centre Managers, and ensuring access to essential resources for the delivery of person-centered care.
Responsibilities
- Coordinate daily Caregiver activities to ensure seamless delivery of care
- Escalate resident care needs to Nurses and Care Centre Managers as required
- Ensure timely access to essential resources for resident care
Requirements
To succeed in this role, you will need:
- A minimum Level 3 National Certificate in Health and Wellbeing or equivalent qualification
- Strong communication skills
- Ability to multitask and juggle tasks effectively
