We provide Ambulance Services and support the health and wellbeing of our community through various programmes.
As a Patient Care Plan Coordinator, you'll play a crucial role in supporting our clinical strategy at a local level by developing and implementing new patient care plans for frequent callers and patients with complex needs.
Your main responsibilities will include:
- Supporting the delivery of our clinical strategy by creating new PCPs for frequent callers and complex needs patients in partnership with local health professionals.
- Working with healthcare professionals, patients and their family/whanau to develop effective and fit-for-purpose PCPs.
- Ensuring all existing PCPs within the region are effective, safe and current by regularly reviewing plans and working with plan owners to update or remove as required.
Your Qualifications
To be successful in this role, you'll need to have an appropriate registered health professional qualification with a current annual practicing certificate and experience in building and maintaining key relationships with local health stakeholders.
You should also have extensive experience in implementing successful patient care plans and specific knowledge of the challenges facing health and social care and the needs of frequent callers and patients with complex needs.
