About This Role
We are seeking a Care Management Specialist to join our team at Access Community Health. As a key member of our team, you will be responsible for overseeing the care coordination of complex care clients and their support teams.

Key Responsibilities
- Manage the rostering of complex care clients and their support teams.
- Collaborate with Support Workers to ensure effective delivery of care services.
- Develop and maintain strong relationships with complex care support teams and clients.
- Support the training and development of the complex care teams.
Requirements
- Proven experience working in care management or a related field.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Current full driver's license.
- Demonstrated commitment to cultural sensitivity and awareness.
