About the Role
The South Adult Community Psychiatric Service is a multidisciplinary team providing assessment, treatment, and support services to individuals with moderate to severe and enduring mental health conditions.
As a Community Mental Health Nurse, you will work collaboratively with a team of professionals, including Tangata Whaiora and their family/whanau, primary care providers, community supports, and inpatient teams.
You will hold a caseload of clients and proactively engage with them to support their treatment and holistic needs. This includes risk assessment, developing recovery plans, and liaison with relevant services.
The ideal candidate will have strong communication skills, be able to think critically, and prioritize tasks effectively.
Key Responsibilities
- To provide high-quality care and support to clients with complex mental health needs
- To develop and implement individualized care plans in collaboration with clients and their families
- To work closely with other healthcare professionals to ensure seamless care transitions
- To maintain accurate and up-to-date records of client interactions and outcomes
About You
We are seeking an experienced and dedicated nurse who is passionate about delivering exceptional patient care.
You will possess excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.
A commitment to ongoing learning and professional development is essential for this role.
Requirements
- A current APC and Mental Health Nursing Scope of Practice
- A Full Driver's Licence
- Competency in Te Reo Maori or a willingness to learn
- Experience working with clients with complex mental health needs
