The opportunity | Te arawatea As a Case Manager, you will be the face of MSD, engaging directly with individuals and whanau to provide financial support, employment services, and housing assistance.
You will support people to achieve their goals by connecting them with the right services and creating tailored plans that work for them.
As the face of the organisation, you will be working alongside whanau to find out what is important to them and to support them into training or employment opportunities.
Key responsibilities include: Building strong relationships and working collaboratively with others to enhance outcomes for whanau.Developing and maintaining relationships with your peers and partner agencies to give and receive support when needed.Engage with individuals to identify barriers and create plans for success.Providing empathetic and professional case management support, maintaining accurate records of client interactions.Managing client progress, adjusting support plans as needed to help them achieve their goals.Navigate various computer systems to manage client information and stay up to date with relevant policies and legislation.If you're dependable, resilient, and passionate about helping our communities, this role could be the perfect fit for you.
This is a permanent opportunity and is based in the Kaikoura Service Centre.
