**Kōrero mō te tūranga - About the role**

FCS(ID) have contractual and statutory reporting obligations to Whaikaha and the Ministry of Health. The Coordinator is a highly rewarding role with a focus on the coordination of services for people with an Intellectual Disability who are managed within the High and Complex Framework (HFC). This is the role for those of you skilled at working with professionals providing options that promote and enable people within the HCF gain maximum function that enables them to have a better quality of life. As part of this phase of their journey, you will be instrumental in the transition of these individuals from the High and Complex Framework to mainstream Disability Services in the Community. This will be the ideal opportunity to demonstrate your sound stakeholder management and communications skills as you manage multiple agencies such as the NASCs and whanau of the individual.

**Moū - About you**

Duties
- Sound understanding of Intellectual Disability and the ability to identify disability needs as part of comprehensive Needs Assessments
- An understanding of how services for people with an intellectual disability are coordinated across New Zealand to aid the function of Service Coordination. It is advantageous to be able to identify all funded and non-funded services and support options
- Demonstrated time management ability to ensure deadlines are met and the Review Process is effectively managed
- Managing documentation-ensuring file notes, needs assessment, service co-ordination, reports, legal documentation is accurate, appropriate, and up-to-date.
