**The details**:Fixed term, Full time or part-time available**Be apart of a kaupapa Māori equity service within Health NZ | Te Whatu Ora, Waikato.**Share your passion to support Māori and Pasifika whānau to engage within the health system and to improve health outcomes for whānau.**Five months full time/part-time role or secondment options available for current Health NZ staff.**

**Te whiwhinga mahi | The Opportunity**

Support whānau to eliminate access barriers to healthcare and bring Pae Ora into the system. Reporting to the Programme Manager you will work within a team of Māori lead Clinicians, Management and Health Navigators. The position is Monday to Friday, 8 hrs shift or part-time is available. Work options include in office and work from home.
- Key responsibilities of the role:
- Undertake activiies as Whānau Hauora Assessment (WHA) Navigator under Equity Hauora WHIRI projects-
- Engage with patients and their whānau, in conducting a WHA assessment-
- Follow up on identified areas of needs as determined by the WHA assesment, ensure required referrals are made and appropriate, education/resources are provided.-
- Enter data into screening tools-
- Assist with contacting patients re appointment reminders for Outpatient Clinics-

**Mōu | About you**
- Skills and experience you’ll need to be considered for the role:
- Experience in implementing Te Tiriti o Waitangi in action-
- Basic understanding of te reo me ona tikanga + understanding of Te Ao Māori and Matauranga Māori-
- Work independently as well as part of a team + excellent communication skills-
- Empathy, ability and confidence to engage people from a range of backgrounds-
