**The Opportunity**
For an Advanced Practitioner Social Worker (PPPR), to join the Post-Acute Community Rehabilitation team. This position is a permanent 40hour pw position. This is a great opportunity to experience working in the hospital environment and the community.
The Post-Acute Community Rehabilitation Service (PACR) is a collective of services primarily dedicated to supporting clients immediately post their acute care episode.
The Advanced Practitioner Social Work Role (PPPR) provides safe and clinically effective comprehensive assessment and intervention, with demonstration of advanced knowledge and skills to manage complex presentations to patients / clients / and their whānau.
To ensure and prioritise a focus on safe and high-quality patient/client care. This includes all interventions and processes within the MidCentral, Whānau and Community, and other health services within MidCentral and wider community as appropriate who require actions that fall under the Protection of Personal and Property Rights Act 1988).
To assist with the co-ordination of MidCentral wide systems, processes and procedures in regards to the Protection of Personal and Property Rights Act 1988.
To provide expertise and to act as a key resource in regards to Protection of Personal and Property Rights Act 1988. This role will have some responsibility for providing clinical leadership across services which assists in developing the clinical capability of others_._
To provide individual and group education to upskill social workers and other health professions across Mid Central to support understanding and awareness of the issues and processes relating to the Protection of Personal and Rights Act 1988.
To provide high quality social work services to individuals, and to their families/whānau or others of their support network, who are referred through the Mid Central PACR service.
The social worker will work as a team member of the Post-Acute Community Rehabilitation Service, being a multi-disciplinary team. The primary role includes supporting patients and whānau with the Protection of Personal Property Rights (3PR) process; and support the transition from inpatient to community. In addition the social worker will support patients discharge from health recovery beds and be involved with stroke and non acute rehabilitation.
The successful applicant is required to have effective oral and interpersonal communication skills, is able to work under pressure and direction, and enjoys meeting the challenges of a varied workload. A willingness to learn and complete ongoing training is necessary. You will need to be aware of principles around person centred practice, be sensitive to cultural differences and committed to the Treaty of Waitangi as well as having a current practicing certificate and drivers licence.
