**The Opportunity**

We are seeking a Social Worker Advanced Practitioner to join the Child Health Community Service, Pa Harakeke: Healthy Women Children and Youth cluster. This is a permanent position, working 0.5 FTE.

The Child Health Community Service is a multidisciplinary team that is community facing but works across the paediatric continuum. This Social Worker role will work alongside the current Social Worker Advanced Practitioner within the Child Health Community team. The role will be based at 200 Broadway Avenue and will cover the geographical area of the MidCentral rohe.
The key focus of the Social Worker is to improve and enhance clinical care of the child and their family/whanau. The Social Worker will act in the roles of a practitioner, educator, change agent and health broker in the pursuit of professional excellence and improved child health outcomes.
The appointee will have appropriate registration with the Social Work Registration Board and a current practicing certificate. Relevant experience in health and /or child-centered practice is required along with appropriate relevant post graduate qualifications or willingness to undertake further postgraduate study.
