An exciting opportunity has become available to work as a Needs Assessment Service Coordinator and join our passionate and multi-disciplinary team at Te Whatu Ora Lakes.
The Needs Assessment Service Co-ordination service undertakes comprehensive health needs assessment and co-ordinates funded and unfunded support services for the Lakes DHB population who have long term, age disability related needs and for people who require short term support following acute event, hospital discharge, or as part of palliative care support.
Reporting to the Team Leader NASC, the Needs Assessor/Service Coordinator will be working across Rotorua Taupo and will be a registered health professional with the following attributes:
- Current New Zealand Practicing Certificate (or equivalent)
- Budgeting and relationship management skills
- Experience in delivering NASC services
- Excellent communication skills
- Experience and affinity with the elderly
- Community and or Disability sector experience
Salary range
$66,750 - $95,340 pro-rated to hours worked.
To be eligible to work at Te Whatu Ora Lakes, you must be fully vaccinated, including COVID-19.
