About us
Comcare is an established Mental Health and Housing provider assisting people with mental health or addiction issues to live well in the community. We provide a rich and diverse range of Mental Health Support Services, Housing and Housing Support Services.
We are looking for a Complex Case Coordinator (Practitioner) to join our Community Integration Service (CIS). The Community Integration Service works with people who have complex social or other issues that create barriers for successful community living and supports them to problem-solve a pathway forward to independence.
About this role
This position is a combination of a service delivery role and developmental role that offers the opportunity for a skilled practitioner with NGO and mental health experience to engage in the more complex transition work of the Community Integration Service, provide clinical guidance to the Community Integration Service alongside the Clinical Team Leader – Community Integration Service.
What you'll be doing
- Undertake more complex transitions for the team.
- Transitions will be well organised, timely, and demonstrate high level skill in creating sustainable support packages for people with complex needs.
- Apply the skill set of your discipline to the work of CIS and Comcare.
- Assist the Clinical Team Leader CIS and Comcare to review and assess risk and any risk mitigation alongside other Comcare clinicians.
- Maintain a caseload.
- Maintain up to date case notes in required format, stats are completed within required timeframes.
- Provide specific assessments or reviews where complex case coordinators feel unable to find a clear path forward.
About you
To be successful in this role, you will have:
- At least three years' experience in community mental health sector.
- A strong ability to create possible options and solutions for individuals who may see themselves as stuck.
- Have skills in collaborative planning for clients with complex needs, thinking outside the box and developing service provision based on a strengths model.
- You will be a motivated and creative individual who has a depth of knowledge of resources in the Canterbury region.
- You will have good communication and strong coordination skills.
- Good facilitation skills.
- Be able to work in a team and individually.
It is desirable that applicants have a Tertiary Qualification in a mental health-related occupation (Nursing, Social Work, Occ. Therapy etc.) or are an
Allied Health Practitioner.
You will need to hold a minimum Certificate in Health and Wellbeing Level 4 - Mental Health and Addiction Support (or equivalent).
Experience in the mental health and disability sector would be an advantage.
Applicants must hold a current full, clean driver's license.
You will have a cultural awareness, knowledge of, and demonstrable commitment to the principles of Te Tiriti o Waitangi.
Eligibility
