Are you a Registered Health Professional with a good understanding of the psycho social needs of people who live with complex mental health issues'

We need someone with a current practising certificate who can act as liaison between our NGO and Te Whatu Ora Waitemata in facilitating complex packages of support for this complex and diverse group who are high users of health services.

This position involves working collaboratively with community partnerships and inter agency supports, so good communication skills are a must Goodwood Park Health Care Group - Mental Health Division provides a variety of residential and community services with individual packages of care.

**The role**:
You will contribute towards effective and innovative methods of enabling and supporting clients in their recovery.
**The core tasks of this role include**:
Service coordination of an individuals supports needs often across multiple agencies/funders, ensuring we are matching people with services that are required and appropriate, the best fit for them.
Responsibility of ensuring that our publicly funded support services are appropriately allocated and utilised and able to evidence this.
Working in partnership with Te Whatu Ora Waitemata Locality Coordination Services.
Completion of Support Needs Assessment and Multi-Agency Plans (SNAP) with individuals currently residing or who have recently resided in the inpatient unit, or are living in the community.
Focus on complex clients with an Axis 1 diagnosis plus other co-existing disorders e.g. Intellectual Disability, Cognitive Disability, Physical Health, AOD, and TBI.
Working with.
Finding and creating innovative and flexible solutions for clients.

**Our ideal Health Professional for this role will possess the following skills and attributes**:
Understand and implement the obligations of Te Tiriti O Waitangi.
The ability to work alongside and model positive outcomes with clients and their family/whnau, through enthusiasm, innovation and a willingness to learn new approaches and techniques.
Able to produce reports/assessments and work to deadlines, demonstrate resilience and maintain a positive attitude.
Three years minimum experience in the mental health field.
An annual practising certificate.
Team oriented and a great communicator.
A full NZ drivers licence.

Proficient computer literacy Microsoft suite.
NZ residency/valid NZ working visa.
