**Kōrero mō te tūranga - About the role**

The purpose of this role is to improve health outcomes for Pacific people admitted to hospital. The role is focused on addressing Pacific health priorities across all ages and to facilitate links to Pacific health providers in the community. The role is critical to support care and treatment for Pacific patients including support for their families and hospital inpatient teams.

**Mō tō mātou rōpū - About our team**

The Pacific health team for inpatients are experienced and skilled staff who understand the cultural needs of Pacific patients and families and also acts as a cultural resource for hospital services. The team provides health assessments and education for patients using a strengths based approach to overcome health literacy where indicated. The role will be based at Wellington hospital but also provides cover to Kenepuru Hospital and Hutt Hospital.
