Kōrero mō te tūranga - About the role
The Specialty Clinical Nurse (SCN) practises in a narrow field but with more in-depth knowledge and skills than a Registered Nurse. This role will enhance health outcomes for clients by working directly with them to provide assessment, care and education within specific area of practice. In addition this role contributes to development of pathways, protocols and guidelines in the specific area of practice. This role requires critical care knowledge which is clinically grounded in expert nursing knowledge.
Supporting change implementation is an important aspect of this role. The practise environment is one of continuing change. The planned introduction of new processes and standards and associated staff education requires collaboration with the senior team to ensure its smooth roll out. The Speciality Clinical Nurse - role needs to be an advocate for the areas / services strategic direction.
Mō tō mātou rōpū - About our team
The VTE Service provides expert education, advice and support and follow up to patients on anticoagulation living in or around the Wellington area. It provides a virtual telephone clinic and nursing support for the VTE clinics. The service receives referrals from inside the hospital and currently there are approximately 300 patients being managed by the service.
The service has policies & protocols in place to support the management of patients on anti-coagulation. It is planned that the service will return many of the stable long term patients back to the general practitioners and focus on the complex acute patients that require careful monitoring. Education packages are given out to support both patients and staff in relation to anticoagulant management.
The SCN role is expected to cover the Haemophilia role when the CNS is on leave. Haemophilia is a disease that affects a small number of patients in New Zealand, however the management and treatment of these patients is expensive. To avoid unfair cost pressures, on the District Health Boards (DHB - s) who have a relatively high number of these expensive patients under their care, the New Zealand Haemophilia Management Group (NZHMG) was created in 2006. The NZHMG receives funding on a per capital basis from all 20 DHB - s and provides specialist commissioning/funding for haemophilia treatment.
The Wellington regional Haemophilia service is part of the Haematology dept. and is currently based in the Wellington Blood and Cancer Centre. It is one of 6 centres that provide specialist comprehensive care to patients with bleeding disorders under the supervision of the NZHMG. The cornerstone of effective bleeding disorder treatment is comprehensive care, delivered by a multidisciplinary team in a specialist haemophilia treatment centre.
