We deliver specialist palliative care across a range of integrated services prioritising care in people’s homes. We provide support during a challenging time in people’s lives.
Our Community Nursing Team provides specialist palliative care support to people living at home with a life limiting illness. They provide a holistic person centred approach to care. Using an integrated approach with primary care our nurses focus on specialist palliative care symptom management and support people to remain at home for as long as they are able. Our Community Nurses are an integral part of our multi discipline team (MDT) working closely with medical staff and our allied health partners.
