This is a fantastic opportunity to utilise your specialist knowledge and skills to provide patient focused care, coordination and liaison with primary and secondary services. This role will primarily focus on respiratory and home oxygen clients. It will involve assessment and education for clients with chronic and complex long term health issues to assist maintain optimal health and wellbeing.

A minimum of 7 years post graduate experience is essential, and a relevant or working towards a post graduate diploma.
