**About the Role**
The primary purpose of the Pathway coordinator is to effectively manage and coordinate multiple clinical pathways and multiple agencies for those clients within the regional forensic and rehabilitation services. The role includes facilitating, coordinating, management, oversight and entry to and discharge from the regional forensic and rehabilitation services. The coordinator will support Group manager in the development of quality improvement initiatives, reporting and coordination Regional Forensics & Inpatient Rehabilitation services as appropriate. The pathway coordinator needs a good understanding and awareness of the current demands on the national and regional resources of Forensic and General adult mental health services.
**About the Team**
We, Te Korowai Whariki, are an operational group of regional and national services that have staff based in various locations across the Central Region from Gisborne, to Whanganui and Wellington. Te Korowai Whariki services are both inpatient and community based.
**About you**
We are seeking a motivated, resourceful and enthusiastic registered health professional to join the Regional Forensics and Rehabilitation inpatient service. An exciting opportunity exists to be part of committed, innovative and create teams implementing multi-disciplinary approach for consumers with the Regional Forensics and Rehabilitation Inpatient Services.
- Has the skill to assess, plan, implement, review and evaluate strategies to ensure quality provision of service delivery.
- Proven experience in facilitating and managing change at a local level.
- Minimum of postgraduate certificate in specialty nursing practice (or undergraduate mental health/learning disability nursing programme)
- Following process (also discerning when a situation may require a slight different process within policy and procedural confines)
- Have excellent communication skills, written, verbal, non-verbal and listening skills.
