**Te whiwhinga mahi | The Opportunity**

The Patient Flow team are at the heart of our improvement journey to achieve efficient & safe patient flow through our hospital.

We are looking for a motivated individual with the skills needed to identify and resolve potential causes of delays to support the timely, safe transition of patients between wards and hospitals. You will be creative in your ability to problem solve while working collaboratively with your team members.

The successful applicant appointed to this Patient Flow Facilitator role, will be responsible for managing acute, elective, inter-district, clinic and day stay flow; forecasting bed demand based on bookings for elective surgery and plans provided by bookers and schedulers, managing the priorities provided by the production co-ordinators or Operating Room Co-ordinators. This role has strong relationships with other Health Districts and private hospitals regarding transfer of patients.

Full on the job training will be provided.

**Please note this is a fully rostered position with a requirement to be available to work weekend and night shifts.**
