Job Summary
We are seeking a highly skilled and compassionate Support Worker to join our team. As a Support Worker, you will provide individualized support to our clients, helping them to develop their independence and acquire skills for daily living. This role requires a strong ability to work flexibly and think creatively to meet the diverse needs of our clients.
Key Responsibilities
The successful candidate will be responsible for:
- Providing individualized support to clients, including assistance with daily living tasks and community activities
- Developing and implementing personalized support plans to meet the unique needs of each client
- Working collaboratively with other staff members to ensure seamless delivery of services
- Monitoring and reporting on client progress and adapting support plans as needed
