About the Role

The ideal candidate will possess excellent organizational skills, empathy, and a willingness to learn. You will work collaboratively with clients, family members, and other healthcare professionals to provide person-centered care that addresses physical, emotional, and social needs.

Key Responsibilities
  • Coordinate care services, including scheduling appointments, managing medications, and arranging transportation
  • Communicate effectively with clients, family members, and other healthcare professionals to ensure seamless care
  • Develop and implement individualized care plans to achieve client goals and outcomes
  • Advocate for clients and families through effective communication and problem-solving
