Role Overview

The Licensed Practical Nurse (LPN) is a licensed professional who provides skilled nursing care and support to patients in their homes. This role requires collaboration with physicians, patients, and other healthcare professionals to develop and implement personalized care plans.

You will be responsible for assessing patient needs, implementing treatment plans, and providing education and support to patients and their families.

Responsibilities

Your key responsibilities will include:

  1. Preparing equipment and supplies effectively and efficiently, adhering to aseptic technique.
  2. Implementing care plans initiated by registered nurses.
  3. Assessing and educating patients and their families about self-care techniques and information relevant to their diagnosis and care plan.
  4. Providing accurate and timely documentation consistent with the care plan.
  5. Participating in case conferences to ensure seamless care delivery.
  6. Providing direct patient care as defined by the State Nurse Practice Act.
  7. Performing vital signs and head-to-toe patient assessments during visits.
  8. Managing and performing wound care treatment, including surgical wounds and wound vacs.
  9. Providing assessment, execution, and management of central lines, peripheral lines, urinary catheters, colostomies, PleurX drains, and lab draws.
