Holistic Needs Assessment Coordinator Expert

We are seeking a highly skilled Needs Assessment Service Coordinator to join our team at Lakes NASC. The successful candidate will be responsible for assessing the needs of clients and coordinating support services to enable them to continue living safely at home.
About You
- You should have a current New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist or Nurse qualification.
- You will need to have relevant professional experience and a full NZ Driver's License.
- Excellent communication skills and the ability to work in partnership with clients and their whanau are essential.
