**About the Role**

An exciting opportunity has become available to work as a holistic support needs specialist and join our passionate and multi-disciplinary team at Lakes NASC. Our work is fast-paced and dynamic, and we offer a positive and supportive team environment which is responsive to clients' needs as well as enabling professional opportunities for learning and development.

The role of NASC is to complete comprehensive assessments of the needs of older adults with health and disability-related needs, and some younger people whose needs relate to a chronic long-term health condition. We also work with clients who have recently discharged from hospital, as well as palliative care clients. Our aim is to support our clients to continue to live safely at home, or enable them to move into Residential Care if needed.

This role involves working with clients across the Lakes region (Rotorua, Mangakino, Taupo, Turangi). Key requirements include having a New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist, or Nurse qualification, along with current APC in appropriate scope and Allied Health or Nursing qualification and relevant professional experience.

**Key Responsibilities**

Conduct comprehensive needs assessments to identify support requirements

Coordinate support services for older adults and younger people with complex needs

Work collaboratively with clients and their whanau to meet support needs

Develop and maintain effective relationships with stakeholders
