Role Description: As a Holistic Needs Assessor at Te Whatu Ora Lakes, you will play a vital role in assessing and addressing the complex needs of our clients. About the Position: The ideal candidate will have a solid understanding of assessment and planning principles, with experience in coordinating care and facilitating access to support services. You will also possess excellent communication and interpersonal skills, enabling effective collaboration with clients, their whānau, and healthcare professionals.

Key Responsibilities:

  • Assess clients' needs, identifying areas of support and facilitation
  • Develop and implement individualized care plans, ensuring alignment with client goals and preferences
  • Coordinate access to necessary support services, including healthcare, social services, and community resources
  • Maintain accurate records and reporting, adhering to relevant guidelines and regulations
  • Participate in regular team meetings, contributing to service improvement initiatives
