Job Summary: We are looking for a Holistic Needs Assessor to join our multidisciplinary team at Te Whatu Ora Lakes. In this role, you will conduct thorough assessments of clients' needs, identify areas of support, and coordinate access to relevant services. Your expertise will enable us to provide person-centered care, empowering individuals to achieve their optimal health and wellbeing.

About the Position: The ideal candidate will possess a strong understanding of social work principles, with experience in assessment, planning, and coordination of care. You will also have excellent communication and interpersonal skills, enabling effective collaboration with clients, their whānau, and healthcare professionals.

Key Accountabilities:

  • Conduct comprehensive assessments of clients' needs, incorporating biological, psychological, and social factors
  • Develop and implement individualized care plans, working in partnership with clients and their families
  • Facilitate access to necessary support services, including healthcare, social services, and community resources
  • Maintain accurate records and reporting, adhering to relevant guidelines and regulations
  • Participate in regular team meetings, contributing to service improvement initiatives
