Job Description Summary

We are seeking a skilled Care Coordinator to join our team at Te Whatu Ora. As a key member of our clinical team, you will work closely with our multidisciplinary team to deliver high-quality care to individuals experiencing early psychosis.

Key responsibilities include:

  • Working collaboratively with the leadership team to develop and implement evidence-based treatment plans;
  • Supporting our rangatahi (young people) and their whānau through their journey, promoting a youth-friendly service;
  • Demonstrating flexibility and excellent communication skills in a fast-paced environment;
  • Maintaining accurate records and reporting on progress to stakeholders.

We are looking for a professional who can bring their expertise and passion for mental health to this rewarding role.
